Schedule: Monday–Friday | 7:00 AM – 3:00 PM or 10pm-6am

We are seeking a dedicated Licensed Practical Nurse (LPN) to support a pediatric patient in a home environment. This role offers the chance to provide personalized, uninterrupted care while working a predictable weekday schedule.

Care Requirements
- Tracheostomy (trach) care
- Ventilator support
- G-tube feedings and medication administration
- Ongoing patient monitoring and assessment
Key Duties
- Provide skilled nursing care tailored to the patient’s needs
- Oversee ventilator function and perform trach care
- Administer enteral feedings and prescribed medications
- Observe and document patient status and any changes
- Maintain a clean and safe care setting
- Partner with the family and clinical team to ensure quality care
Required Qualifications
- Current Ohio LPN license
- Experience with ventilator, trach, and G-tube care preferred (Training available)
- Valid CPR certification
- Reliable transportation and valid driver’s license
- Recent TB test or chest X-ray
- Ability to pass background check and drug screening
- Strong reliability and communication skills
