We are seeking a Licensed Practical Nurse (LPN) to join our team and work closely with our case management staff to support coordinated, patient-centered care.
Key Responsibilities:
- Assist with patient assessments and monitoring
- Collaborate with case managers and care team members
- Perform medication set-ups (med-set ups) and administer medications as ordered
- Document patient status and report changes
- Educate patients and caregivers on care plans and medications
Qualifications:
- Active LPN license in good standing with the Indiana State Board of Nursing
- Compliance with Indiana licensing requirements (IPLA), including renewal and scope of practice
- Strong communication, organization, and teamwork skills
- Experience in home health or case management preferred

Compensation: $35.00 - $38.00 per hour
