About the Role:
Join MedPOINT Management as an Inpatient UM Nurse Case Manager in Sherman Oaks, CA, where you will play a vital role in patient care coordination. Be part of a dynamic team dedicated to improving health outcomes and ensuring high-quality care for our patients.
Responsibilities:
- Conduct comprehensive assessments of patients to determine care needs.
- Collaborate with healthcare providers to develop and implement care plans.
- Monitor patient progress and adjust care plans as necessary.
- Facilitate communication between patients, families, and healthcare teams.
- Ensure compliance with regulatory and accreditation standards.
- Educate patients and families about treatment options and resources.
- Document all case management activities in accordance with policies.
- Participate in quality improvement initiatives and team meetings.
Requirements:
- Active RN license in California with a minimum of 2 years nursing experience.
- Experience in case management or utilization management preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a fast-paced environment.
- Knowledge of healthcare regulations and managed care principles.
- Certification in Case Management (CCM) is a plus.
- Proficient in electronic health records and documentation practices.

Compensation: $38.00 - $42.00 per hour
