Now Hiring: Home & Community-Based Case Manager
We are seeking a compassionate and dedicated Home & Community-Based Case Manager to join our team. In this role, you will support individuals in maintaining independence and enhancing their quality of life within their homes and communities.

Key Responsibilities:

  •  Coordinate and monitor individualized care plans 
  •  Connect clients with appropriate community resources and services 
  •  Advocate for client needs while promoting dignity and independence 
  •  Maintains and promotes client satisfaction 
  •  Provides leadership and oversight to direct and indirect care team members, ensuring productivity and performance align with agency policies and procedures 
  •  Participate in and assist with the on-call schedule as needed 
  • Competently and accurately completes assigned tasks within established guidelines and time frames according to Agency policy and procedure. 
  •  Perform additional duties as assigned by the immediate supervisor 
Qualifications:

  • Experience in case management, social services, or healthcare preferred 
  •  Dependable, punctual, and self-motivated 
  •  Strong communication and organizational skills 
  •  Passion for helping others and making a meaningful impact
