About the Role:
As a Case Manager at Children's Advocate LLC, you will play a vital role in supporting children and families in need. Join our dedicated team in Frankfort, IL, and make a meaningful impact by guiding clients through challenging situations with compassion and expertise.
Responsibilities:
- Conduct comprehensive assessments to identify client needs and develop tailored service plans.
- Provide ongoing support and advocacy for children and families in various situations.
- Collaborate with community resources and service providers to ensure holistic care.
- Maintain accurate case documentation and track progress towards goals.
- Facilitate family meetings and coordinate services among multiple stakeholders.
- Monitor client progress and adjust service plans as necessary.
- Educate families about available resources and support systems.
- Participate in training and professional development opportunities to enhance skills.
Requirements:
- Bachelor’s degree in Social Work, Psychology, or a related field.
- Minimum of 2 years of experience in case management or social services.
- Strong communication and interpersonal skills for effective client interaction.
- Knowledge of child welfare laws and community resources.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate demeanor with a commitment to advocating for children and families.
- Valid driver's license and reliable transportation for client visits.
- Proficiency in case management software and documentation practices.

Compensation: $24.00 - $26.00 per hour
