We are looking for a well-connected RN who wants to turn their existing healthcare relationships into a recurring income stream.
If you already have connections with:
- Hospital discharge planners
- Skilled Nursing Facilities (SNFs)
- Case managers
- Assisted Living communities
You can monetize those relationships immediately.
What You’ll Do
- Refer private-pay and long-term insurance clients to HomeWell Care Services
- Build and maintain your referral network
- Help families transition safely from facility to home
- Collaborate with our internal team to ensure quality care
What You Earn
- 5%–8% recurring commission on every client you bring in
- Monthly income for the life of the case
- Bonuses for high performers

- 10 clients → $3K–$5K/month
- 20 clients → $6K–$10K+/month
