About the Role:
Join Skilled Nursing Pharmacy as a Pharmacist Consultant, where you will play a pivotal role in enhancing patient care through expert medication management. Responsibilities:
- Conduct comprehensive medication reviews and assessments for skilled nursing facilities.
- Provide expert consultation to healthcare teams on medication therapy management.
- Develop and implement individualized care plans for residents.
- Monitor patient outcomes and adjust medication regimens as necessary.
- Educate staff and patients on medication safety and efficacy.
- Participate in interdisciplinary team meetings to discuss patient care strategies.
- Stay current with pharmacy laws and regulations to ensure compliance.
- Build strong relationships with clients and healthcare providers to foster collaboration.
Requirements:
- PharmD degree from an accredited pharmacy school.
- Current Washington State pharmacist license in good standing.
- Minimum of 2 years of experience in a clinical or consulting pharmacy role.
- Strong knowledge of pharmacotherapy and medication management.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Detail-oriented with strong analytical skills.
- Commitment to ongoing professional development and learning.

Compensation: $130,000.00 - $135,000.00 per year
