HomeWell Care Services of NW Indiana wants to consider you for our team! Looking for something where no day is the same that has some flexibility for your life? We are seeking a dedicated, Care Manager to join our team ... one who is professional and trustworthy, able to take charge from the start, a quick learner, organized, able to multi-task, compassionate, and well-spoken. The Care Manager, will be our liaison between intra-office staff, caregivers, clients, and family, therefore great communication skills are integral to the position. We are looking for someone who is an uplifting team player that has the drive and focus for growth, with a "can do" attitude. 

MINIMUM QUALIFICATIONS:
  • Home care or health care industry experience preferred, though not mandatory depending on knowledge base
  • 1-2 years experience in an office environment 
  • Knowledge of medical terminology, marketing/sales, basic recruiting, and/or scheduling
  • Must have a reliable means of transportation with current insurance and a valid, unrestricted driver’s license. 
  • Solid computer skills (Microsoft office), attention to detail and grammar, and be able to learn new applications/platforms with ease.
  • Must pass a national/expanded criminal background check. 
  • Excellent organizational skills as well as attention to detail and be able to multitask, exercise initiative, problem solve and apply sound judgment.
MAIN DUTIES AND RESPONSIBILITIES:
  • Grow and manage the recruiting and interviewing of care staff candidates
  • Lead and manage caregiver orientation, including updates to program as needed
  • Diligently maintain detailed reports, complete data entry
  • Complete assessments as needed 
  • Develop and implement detailed written care plans
  • Assign and schedule caregivers to clients
  • Manage call-offs to ensure client coverage
  • Regularly communicate with clients, families, designated client representatives
  • Supervise caregivers
  • Attend and host health fairs and job fairs as needed
  • Perform scheduled and non-scheduled in-home client supervisory visits
  • Complete additional duties and responsibilities as assigned by management and as needed
