Responsibilities:
- Conduct comprehensive assessments to evaluate patients' physical abilities and needs.
- Develop personalized treatment plans aimed at achieving individual rehabilitation goals.
- Implement therapeutic exercises and modalities to enhance mobility and reduce pain.
- Educate patients and families on treatment strategies and home exercise programs.
- Monitor progress and adjust treatment plans as necessary to ensure optimal outcomes.
- Collaborate with other healthcare professionals to provide integrated care.
- Maintain accurate documentation and records of patient progress and treatment.
- Stay updated on the latest therapeutic techniques and industry best practices.
Requirements:
- Valid state license to practice as a Physical Therapist in Connecticut.
- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
- Experience in home health care or outpatient settings is preferred.
- Strong communication and interpersonal skills to engage with patients and families.
- Ability to work independently and as part of a collaborative team.
- Compassionate demeanor with a commitment to patient-centered care.
- Knowledge of current evidence-based practices in physical therapy.
- CPR and First Aid certification is a plus.
