About the Role:
Join Skilled Nursing Pharmacy as a Staff Pharmacist and play a vital role in providing exceptional pharmaceutical care to our patients. In this dynamic position, you'll collaborate with a dedicated team to ensure optimal medication management and patient safety.
Responsibilities:
- Dispense medications accurately and efficiently in a skilled nursing environment.
- Conduct medication therapy management and patient counseling.
- Collaborate with healthcare providers to optimize patient medication regimens.
- Review and verify prescriptions for accuracy and appropriateness.
- Monitor patient outcomes and adjust therapies as needed.
- Educate nursing staff on medication administration and safety protocols.
- Maintain compliance with pharmacy regulations and quality standards.
- Participate in continuous professional development and training initiatives.
Requirements:
- PharmD degree from an accredited pharmacy school.
- Active Washington State pharmacist license.
- Minimum of 1 year experience in a clinical or skilled nursing setting preferred.
- Strong communication and interpersonal skills to foster teamwork.
- Detail-oriented with a commitment to patient safety and quality care.
- Ability to manage multiple tasks in a fast-paced environment.
- Knowledge of pharmacy software and electronic health records.
- Passion for improving patient outcomes through pharmaceutical care.
