Mental Health Resource Center is a not-for-profit Florida corporation that provides a wide range of mental health and behavioral health care services to the community. Services include 24-hour emergency services, inpatient psychiatric services for children, adolescents, and adults, as well as outpatient services such as medication management, case management, and counseling.
Job Summary
As a member of the Crisis Response Care Coordination team, the Crisis Response Care Coordinator works with individuals identified by the MHRC Co-Responder Program and/or the Jacksonville Sheriff's Office (JSO) Behavioral Crisis Response Unit following a crisis event or law enforcement encounter. The Care Coordinator provides outreach, assessment, care coordination, and linkage to community-based services to help individuals access ongoing treatment and supports. This position also assists with transition planning for individuals being discharged from inpatient facilities or incarceration and supports community outreach efforts for individuals identified as being at risk for suicide.
Responsibilities
- Provides crisis response care coordination services and follow-up to individuals identified through the Co-Responder Program and JSO Behavioral Crisis Response Unit.
- Conducts outreach and engagement efforts to connect individuals with behavioral health, medical, housing, and community support services.
- Completes suicide risk screenings, safety plans, biopsychosocial assessments, and other required assessments.
- Develops individualized care plans and/or treatment plans in collaboration with individuals served, family members, and service providers.
- Facilitates warm handoffs and referrals to internal and external treatment providers and community resources.
- Monitors individuals' progress and the effectiveness of services provided through ongoing follow-up contacts.
- Assists eligible individuals with enrollment into MHRC services and coordinates access to community resources and supports.
- Provides outreach and transition planning services for individuals being released from inpatient psychiatric facilities, Crisis Stabilization Units (CSUs), or incarceration.
- Completes face-to-face contacts and follow-up activities in accordance with program requirements.
- Participates in JSO roll calls, trainings, facility tours, and collaborative meetings to strengthen partnerships and improve service coordination.
- Provides timely case updates to JSO officers, Co-Responder clinicians, and other treatment team members.
- Maintains accurate, timely, and comprehensive documentation within the Electronic Health Record (EHR).
- Compiles reports and data as assigned to support program performance and reporting requirements.
- Assesses and monitors for risk, trauma, abuse, neglect, and other safety concerns, utilizing appropriate reporting procedures when necessary.
- Serves as an advocate for individuals served and promotes recovery-oriented, person-centered care.
- Participates in community outreach activities to increase awareness of crisis response and care coordination services.

- Transforms brief notes or session conversations into complete, clinically appropriate documentation
- Supports accurate, compliant documentation while allowing staff to review and finalize notes
- Provides helpful insights from sessions to support care planning and continuity
- Accessible on desktop or mobile for flexibility across different work settings
Qualifications
- In order to be considered, a candidate must have Bachelor’s degree in Social Work or a related Human Services field from an accredited university or college (a related Human Services field is defined as one in which 30 hours of course work includes the study of human behavior and development) required.
- One year of experience working in human services or a mental health related field required. Experience working with adult individuals with mental illness preferred.
- Proficiency in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System demonstrated within three months of employment.
- Proficiency in Microsoft Office, Outlook and the use of the Internet.
- Strong communication skills are essential and this individual must be able to interact appropriately with internal and external customers, families, caregivers, community service providers, supervisory staff and other department professionals.

Mental Health Resource Center, Inc. (MHRC) is a nonprofit organization that provides a wide range of behavioral health and social services to the community. Included in MHRC’s service array are 24-hour emergency services and inpatient psychiatric services for children, adolescents, and adults. Outpatient services include medication management, care coordination, case management, intensive case management team services, counseling, psychosocial rehabilitation, an adult therapeutic family program, state hospital liaison services, mental health court, multidisciplinary forensic teams (MFT), jail-based diversion services, a Co-Responder Program with the Jacksonville Sheriff’s Office, the Link-to-Life suicide prevention program, and services that assist individuals in obtaining benefits such as Social Security Income, Social Security Disability Income, Medicaid, food stamps, and housing. In addition, UF Health Jacksonville contracts with MHRC to manage the hospital’s adult inpatient psychiatric unit.
MHRC also operates eight Florida Assertive Community Treatment (FACT) programs for adults with severe and persistent mental illness, located in Clearwater, Gainesville, Jacksonville, Kissimmee, Rockledge, Tampa, and Winter Haven, as well as a FACT Lite program providing Linking, Advocating, Treating, Transitioning, Empowering & Recovery Support (LATTERS) services in Jacksonville. The two FACT teams located in Jacksonville serve residents of Duval, Clay, and Nassau counties.
