Case Manager Housing Navigator assists Medi-Cal members experiencing homelessness or at
risk of homelessness in obtaining and maintaining permanent housing through California's
CalAIM Community Supports Program. The Housing Navigator serves as the primary liaison
between members, landlords, property management companies, housing authorities, and
community resources to remove barriers to housing stability.
Essential Duties
Housing Navigation
Conduct housing needs assessments.
Develop individualized housing plans.
Locate affordable housing opportunities.
Maintain relationships with landlords and property managers.
Coordinate apartment tours and housing applications.
Assist members with lease documentation.
Advocate for members during the housing search process.
Housing Deposits
Coordinate security deposits.
Coordinate utility deposits.
Obtain required landlord documentation.
Process move-in assistance requests.
Verify member eligibility.
Maintain payment documentation and receipts.
Housing Transition Services
Coordinate move-in dates.
Assist with obtaining furniture and essential household items.
Coordinate utility activation.
Connect members with community resources.
Ensure successful transition into permanent housing
Housing Tenancy Services
Conduct home visits.
Monitor housing stability.
Assist with landlord disputes.
Provide eviction prevention services.
Teach budgeting and lease compliance.
Develop housing stability plans.
Coordinate with ECM providers.
Financial Literacy & Life Skills Facilitation
Facilitate individual and group classes on budgeting, financial literacy, credit building,
banking, and money management.
Educate members on developing and maintaining a household budget.
Teach members how to manage rent, utilities, and other recurring household expenses.
Provide instruction on building and repairing credit, understanding credit reports, and
improving financial stability.
Assist members with establishing savings goals and developing long-term financial plans
to support housing stability.
Facilitate life skills workshops focused on independent living, lease compliance, tenant
responsibilities, and self-sufficiency.
Document attendance, participation, and member progress in accordance with
Community Supports documentation requirements.
Care Coordination
Collaborate with ECM Care Managers.
Participate in case conferences.
Communicate with medical and behavioral health providers.
Coordinate referrals.
Maintain regular contact with members.
Documentation
Complete all Community Supports documentation within required timeframes.
Document services in the electronic health record.
Maintain landlord communication logs.
Upload receipts and supporting documentation.
Ensure compliance with DHCS and IEHP requirements.
Minimum Qualifications
Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Public
Health, or related field preferred.
Associate degree with relevant experience may be considered.
Minimum of 2 years of housing navigation, homeless services, case management, or
supportive housing experience.
Experience with Medi-Cal, CalAIM, or Community Supports preferred.
Valid California driver's license and clean driving record.
