Ritechoice Family Services Inc is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing healthcare team in Philadelphia, PA. Responsibilities:
- Provide direct patient care including monitoring vital signs, administering medications, and wound care
- Assist in developing and implementing individualized patient care plans
- Collaborate with RNs, physicians, and interdisciplinary care teams to ensure quality outcomes
- Document patient assessments, treatments, and progress accurately in medical records
- Educate patients and families on health management, medications, and discharge instructions
- Respond promptly to patient needs and changes in condition, escalating concerns as appropriate
- Maintain compliance with all state, federal, and organizational healthcare regulations
Requirements:
- Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania
- Graduate of an accredited LPN/LVN nursing program
- Current CPR/BLS certification
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills with patients and care teams
- Ability to work independently and as part of a multidisciplinary team
- Compassionate, patient-focused demeanor with a commitment to quality care
