About the Role:
Medical Providers Partners PLLC is looking for a dedicated and compassionate Nurse Practitioner. This role is well-suited for early- to mid-career clinicians looking for meaningful work and a collaborative clinical environment.
You will provide comprehensive medical care to approximately 20 patients per day. This position offers autonomy, consistency, and the opportunity to build strong clinical relationships.
Responsibilities – Skilled Nursing Facility Provider
- Provide assessment, diagnosis, and management of acute and chronic conditions
- Conduct regular patient rounding and timely evaluations
- Order and interpret labs, diagnostics, and imaging
- Prescribe and manage medications in compliance with regulations
- Maintain accurate, timely documentation in the EMR
- Collaborate with nursing, rehab, pharmacy, hospice, and administrative teams
- Respond to change-in-condition and urgent care needs
- Support advance care planning and end-of-life discussions
- Participate in quality assurance and regulatory readiness efforts
Qualifications
- Active Pennsylvania Nurse Practitioner license and required certifications
- Graduation from an accredited Nurse Practitioner program
- Strong attention to detail and timely documentation practices
- Self-motivated, dependable, and patient-centered
- Willingness to participate in a rotating on-call schedule
Compensation: $100,000.00 - $120,000.00 per year
