About the Role:
Sky Heart Home Care Services is looking for a compassionate and reliable Care Transport Associate Driver (NEMT) to join our growing team in Smithfield, NC. In this role, you'll make a meaningful difference every day by safely transporting clients to and from medical appointments, ensuring they receive the care they need. Responsibilities:
- Safely transport non-emergency medical transportation (NEMT) clients to and from medical appointments, dialysis centers, therapy sessions, and other healthcare facilities
- Assist passengers with boarding, exiting, and securing wheelchairs, walkers, and other mobility devices
- Maintain accurate trip logs, mileage records, and client documentation in compliance with NEMT regulations
- Conduct pre-trip and post-trip vehicle inspections to ensure safety and cleanliness standards are met
- Communicate effectively with dispatchers, clients, and healthcare providers regarding scheduling and trip updates
- Adhere to all traffic laws, company policies, and HIPAA confidentiality requirements
- Provide courteous, patient-centered service to elderly, disabled, and medically vulnerable passengers
Requirements:
- Valid driver's license with a clean driving record
- Previous NEMT, medical transport, or passenger transport experience preferred
- CPR and First Aid certification (or willingness to obtain)
- Ability to operate wheelchair lifts, ramps, and securement systems
- Strong punctuality, time management, and organizational skills
- Compassionate, patient, and professional demeanor when working with vulnerable populations
- Ability to pass a background check and drug screening

When you face an overwhelming personal challenge, life-altering changes can result. This was the situation for founder Tasha Jones. Several years ago she experienced such a crisis when faced with the loss of her young daughter, after a cardiac issue proved fatal.
During the time she spent in hospital, Mrs. Jones discovered the need for and benefits of quality care. She didn’t fully know it at the time, but the spark to create Sky Heart Home Care Services was lit during this immensely difficult period in her family’s life.
Driven by a need to help support other families during times of need—whether a crisis, or simple daily support and love—it wasn’t long before Mrs. Jones decided to bring together others who shared her mission. Building off her own 15+-year career in mental and behavioral health, and facility management, she launched her own home care service.
She named it Sky Heart in honor of her daughter, Skylar.
