Registered Nurse RN On-Call

- Provide on-call nursing coverage for one designated week each month, including evenings, weekends, and holidays as scheduled.
- Respond promptly to patient, caregiver, and staff calls regarding changes in condition, symptom management, medication concerns, and care needs.
- Perform telephone triage and determine appropriate interventions based on patient needs.
- Conduct skilled nursing visits when clinically indicated.
- Coordinate care with physicians, case managers, therapists, hospice staff, and other healthcare professionals.
- Assess, implement, and evaluate individualized patient care plans.
- Administer medications and monitor for effectiveness and adverse reactions as needed.
- Educate patients and caregivers regarding disease processes, medication management, safety, symptom control, and available resources.
- Document assessments, interventions, communications, and changes in patient condition accurately and timely.
- Report significant findings and concerns to the appropriate provider and supervisor.
- Maintain compliance with agency policies, Medicare Conditions of Participation, and professional nursing standards.
- Current, active, and unrestricted Utah Registered Nurse (RN) license.
- Home Health and/or Hospice experience preferred.
- Excellent assessment, critical thinking, and clinical decision-making skills.
- Strong communication and interpersonal skills.
- Ability to work independently and manage urgent situations effectively.
- Reliable transportation, valid driver's license, and current auto insurance.
- Current CPR certification required.
- Proficiency with electronic medical records and documentation systems preferred.
- Ability to travel throughout Utah County and Salt Lake County as needed.
- On-Call Coverage: 7 days on / 7 days off
- Additional visit opportunities may be available based on agency needs and employee preference.
