Make an Impact in Care Management
We are seeking an experienced RN Manager – Care Management to lead and support a collaborative care management team focused on delivering safe, coordinated, and patient-centered care.
This is an excellent opportunity for an experienced nursing leader with a strong background in acute-care case management, care coordination, and transitional care to make a meaningful impact on patient outcomes while helping improve the overall care delivery experience.
What You'll Do
- Provide leadership and operational support for Care Management and Case Management services.
- Lead collaborative efforts involving nurses, social workers, physicians, patients, caregivers, payers, and community partners.
- Promote effective care coordination and transitional care management across the continuum.
- Support appropriate utilization of healthcare resources and facilitate timely, safe patient transitions.
- Help address complex barriers to care, including social determinants of health, psychosocial challenges, mental health, substance use, financial/resource limitations, and other patient needs.
- Foster strong interdisciplinary communication and collaboration.
- Promote patient self-determination and individualized care planning.
- Support quality improvement, regulatory compliance, and effective care management practices.
- Lead, mentor, and develop care management staff while promoting a culture of teamwork and excellence.
What We're Looking For
Required:
- Bachelor's Degree in Nursing (BSN)
- Active California Registered Nurse (RN) License upon hire
- Minimum 5 years of healthcare experience
- Minimum 3 years of Case Management experience in an acute-care setting
- Minimum 1 year of healthcare leadership experience
Preferred:
- Master's degree in Healthcare, Nursing, or a related field

Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification
