Service Area: Atlantic, Burlington, Camden, Somerset & Gloucester Counties
Pay Rate: Per Diem, TBD
Schedule: Daytime Shift

Priority Group Services is looking for a compassionate, organized, and dependable Support Coordinator to join our DDD team.

As a Support Coordinator, you will help individuals access needed services and supports, coordinate with families and service providers, monitor services, and ensure that services are provided according to each individual's needs and Individualized Service Plan (ISP).

Priority Group Services provides staffing and support services throughout New Jersey, including Home Care and Developmental Disabilities (DDD) services.

Our services include Home Care, Personal Care, Companionship, Clinical Support, Care Management, and DDD Community-Based Services.

Job Responsibilities

Service Coordination

  • Coordinate services and supports for adults with developmental and/or intellectual disabilities.

  • Work with individuals, families, service providers, and community resources.

  • Assist individuals in identifying their needs, goals, preferences, and desired outcomes.

  • Help individuals access appropriate community and support services.

ISP and Documentation

  • Assist with the development, implementation, and monitoring of Individualized Service Plans (ISPs).

  • Maintain accurate and timely documentation.

  • Record contacts, meetings, services, and follow-up activities.

  • Ensure documentation is complete and meets DDD requirements.

Communication and Follow-Up

  • Communicate with individuals, families, and service providers.

  • Follow up on services, appointments, referrals, and concerns.

  • Monitor whether services are being provided as outlined in the ISP.

  • Identify service gaps and assist with finding appropriate solutions.

Monitoring and Advocacy

  • Monitor individual progress toward goals.

  • Advocate for the individual's needs, choices, and preferences.

  • Assist with resolving concerns related to services and supports.

  • Maintain regular communication with individuals and their support teams.

Compliance

  • Follow DDD policies, procedures, and requirements.

  • Maintain confidentiality and comply with HIPAA and privacy regulations.

  • Keep records organized and audit-ready.

  • Complete required reports and documentation on time.

Qualifications

  • Bachelor's degree in Human Services, Social Work, Psychology, Sociology, or a related field preferred.

  • Experience in social services, case management, care coordination, or a related field preferred.

  • Experience working with adults with developmental and/or intellectual disabilities preferred.

  • Knowledge of DDD services and community resources is a plus.

  • Strong organizational and time-management skills.

  • Excellent written and verbal communication skills.

  • Ability to manage multiple responsibilities and deadlines.

  • Proficiency with Microsoft Office and database systems.

  • Ability to work independently and as part of a team.

  • Ability to maintain confidentiality and follow HIPAA requirements.

Experience with iRecord, ISP development, and DDD services is a plus.

Bilingual candidates, including Spanish or ASL, are encouraged to apply.

DDD Compliance Requirement

Candidates with previous experience working with individuals with developmental and/or intellectual disabilities must be able to provide employment history that clearly documents:

  • Experience working directly with adults with DD/ID.

  • Hours worked per day and days worked per week.

  • Total length of employment or service.

  • The organization's mission and connection to DD/ID services.

  • Responsibilities and services provided to individuals with DD/ID.
