Position Summary: The Clinical Case Manager is responsible for assisting the Clinical Director in coordinating treatment services including initial and ongoing coordination of care, advocacy, behavior intervention, aftercare, follow-up, and discharge planning. Case Management will work directly under the Clinical Director to ensure all policies and procedures regarding client care, discharge planning, and case management service needs.
Essential Functions
- Conduct comprehensive assessments to identify clients' needs and develop individualized care plans.
- Coordinate and advocate for the delivery of a range of services to meet clients' substance abuse treatment goals.
- Collaborate with Clinical Director, healthcare team and external agencies to ensure continuity of care.
- Provide support and guidance to clients in accessing community resources and support services.
- Document all notes into EMR accurately and complete as it pertains to interaction with patients and the communication of their discharge plan and aftercare schedule.
- Facilitate communication and collaboration among the treatment team to ensure a cohesive and person-centered approach.
- Monitor and evaluate clients' progress toward treatment goals to assist with aftercare planning.
- Participate in team meetings to discuss client care and treatment plans.
- Provide crisis intervention and support as needed.
- Performs other duties as assigned by the supervisor, which are consistent with the position and in compliance with agency policies and procedures.
Qualifications
- Bachelor’s degree in social work, human services, or a related field, preferred.
- Proven experience in case management, preferably in a mental health or substance abuse setting
- Strong organizational and communication skills
- Ability to collaborate with a multidisciplinary team and external agencies
- Knowledge of community resources and support services
- Commitment to promoting a culture of diversity, equity, and inclusion
- Previous experience in addiction treatment setting or with treatment of co-occurring mental health and substance use disorders preferred
Compensation: $38,000.00 - $40,000.00 per year
