About the Role:
Solara Hospice LLC is seeking a compassionate and dedicated Hospice On Call RN to join our team in Waltham, MA. This is a meaningful opportunity to provide expert clinical care and emotional support to patients and families during life's most important moments. Responsibilities:
- Respond to after-hours patient and family calls, providing clinical guidance and triage support
- Conduct urgent visits to assess and manage acute changes in patient condition
- Administer and adjust medications for pain management and symptom control
- Coordinate with the interdisciplinary hospice team to ensure continuity of care
- Document patient assessments, interventions, and outcomes accurately in the EMR system
- Provide compassionate end-of-life support and education to patients and their families
- Communicate critical updates to attending physicians and on-call medical staff as needed
Requirements:
- Active RN license in the state of Massachusetts
- Minimum 1-2 years of hospice, palliative care, or home health nursing experience preferred
- Strong clinical assessment and critical thinking skills
- Experience with pain and symptom management in end-of-life care
- Proficiency with electronic medical records (EMR) documentation
- Valid driver's license and reliable transportation for home visits
- Excellent communication skills and ability to remain calm under pressure
- Compassionate, patient-centered approach to care
