About the Role:
Prestige Home Health Care LLC is looking for a compassionate and skilled Registered Nurse (RN) to join our dedicated team in Phoenix, AZ. Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, treatments, and skilled nursing interventions in the home setting
- Monitor and document patient progress, vital signs, and changes in condition
- Coordinate care with physicians, therapists, and other healthcare team members
- Educate patients and family members on disease management, medications, and home safety
- Perform wound care, IV therapy, catheter care, and other skilled nursing procedures
- Ensure compliance with home health regulations, policies, and documentation standards
Requirements:
- Active Registered Nurse (RN) license in the state of Arizona
- Current CPR/BLS certification required
- Minimum 1 year of clinical nursing experience; home health experience preferred
- Strong knowledge of skilled nursing procedures, wound care, and medication management
- Excellent communication, time management, and critical thinking skills
- Valid driver's license and reliable transportation for home visits
- Ability to work independently and manage a patient caseload efficiently
