We are looking for a passionate and driven Community Outreach Coordinator to join our team in Glendale, AZ. Responsibilities:
- Develop and execute community outreach strategies to identify and recruit eligible patients
- Build and maintain relationships with community organizations, healthcare providers, and local partners
- Conduct outreach activities at community events, health fairs, and partner locations
- Screen and enroll prospective patients into programs, ensuring eligibility requirements are met
- Maintain accurate records of outreach activities, patient contacts, and recruitment metrics
- Serve as a trusted resource and point of contact for patients navigating available services
- Collaborate with internal teams to align outreach efforts with organizational goals
Requirements:
- Experience in community outreach, patient recruitment, or a related healthcare/social services role
- Strong interpersonal and communication skills with the ability to connect with diverse populations
- Knowledge of local Glendale/Phoenix Metro community resources and healthcare landscape is a plus
- Ability to work independently, manage time effectively, and meet recruitment goals
- Proficiency in maintaining accurate documentation and data entry
- Bilingual (English/Spanish) preferred but not required
- Valid driver's license and reliable transportation for community-based activities
