About the Role:
North Point Senior Homecare in Virginia Beach, VA is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team on a PRN basis. Responsibilities:
- Provide direct patient care to senior clients in a home care setting under the supervision of a Registered Nurse or physician
- Administer medications, treatments, and injections as prescribed and document accordingly
- Monitor and record patient vital signs, health status, and changes in condition
- Assist with wound care, catheter care, and other skilled nursing procedures
- Communicate effectively with patients, families, and the care team regarding patient progress and needs
- Maintain accurate and timely clinical documentation in compliance with state and agency regulations
- Educate patients and caregivers on health management, medications, and care plans
Requirements:
- Active Licensed Practical Nurse (LPN) license in the state of Virginia
- Previous home care, geriatric, or long-term care nursing experience preferred
- Current CPR/BLS certification
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills with a patient-centered approach
- Reliable transportation and valid driver's license for home visits
- Ability to work independently with minimal supervision in a home care environment
