About the Role:
APremium Healthcare Solution, LLC is looking for a dedicated and compassionate Remote Patient/Clinical Coordinator to join our growing team in Conyers, GA. In this role, you'll be the vital link between patients and clinical care teams, ensuring seamless coordination and an exceptional patient experience — all from the comfort of a remote setting.
Responsibilities:
- Coordinate patient care activities and communicate effectively between patients, providers, and clinical staff
- Schedule and manage patient appointments, follow-ups, and referrals
- Maintain accurate and up-to-date patient records within electronic health record (EHR) systems
- Monitor patient progress and proactively address care gaps or scheduling conflicts
- Provide patient education and support, ensuring understanding of care plans and next steps
- Collaborate with clinical teams to ensure compliance with care protocols and regulatory standards
- Handle patient inquiries, concerns, and escalations with professionalism and empathy
Requirements:
- Prior experience in a patient coordination, clinical coordination, or healthcare administrative role
- Familiarity with EHR/EMR platforms and healthcare management software
- Strong knowledge of medical terminology and clinical workflows
- Excellent communication and interpersonal skills with a patient-first mindset
- Ability to multitask, prioritize, and work independently in a remote environment
- High school diploma or equivalent required; associate's or bachelor's degree in healthcare or related field preferred
- Reliable internet connection and a dedicated, distraction-free home workspace

Compensation: $65,000.00 - $75,000.00 per year
