Job Summary:

Aveon Services is seeking a Licensed Practical Nurse (LPN) to conduct in-home assessments, supervise care staff, and provide ongoing training and support to ensure high-quality care for our clients. This role is ideal for nurses who are passionate about leadership, education, and making a meaningful impact in home healthcare.

Key Responsibilities:

  • Conduct comprehensive in-home nursing assessments for new and existing clients
  • Develop and update care plans in coordination with RNs and the care team
  • Supervise, evaluate, and support caregivers (CNAs, CMTs, PCAs) assigned to clients
  • Provide onboarding and ongoing training to field staff on protocols, documentation, and client care
  • Monitor care delivery and ensure adherence to care plans and agency standards
  • Serve as a liaison between clients, families, caregivers, and agency leadership
  • Report changes in client condition or staff concerns to the RN or Administrator
 Duties Include:

  • Perform intake assessments, routine reassessments, and client check-ins in the home
  • Review care documentation submitted by staff; provide feedback and coaching as needed
  • Lead skills-based training sessions (e.g., ADLs, infection control, documentation)
  • Observe caregiver performance and provide field-based evaluations
  • Ensure compliance with infection control procedures and HIPAA regulations
  • Collaborate with the administrative team to continuously improve care quality
  • Respond to emergencies promptly and escalate per agency protocol
 Qualifications:

  • Active LPN license with the Maryland Board of Nursing (Required)
  • Current CPR and First Aid certification (Required)
  • Physical exam and TB test completed within the past 12 months
  • Valid photo ID and Social Security card (front and back copies)
  • Minimum 1 year of LPN experience (home care or supervisory experience preferred)
  • Experience in caregiver training and staff development is a strong plus
  • Clean background check
  • Excellent communication, leadership, and documentation skills
  • Reliable transportation and the ability to travel throughout Maryland

Compensation: $24.00 - $34.00 per hour
