About the Role

The Community Relations Liaison builds and maintains strong relationships with clients, caregivers, and referral partners to grow referral volume, client hours, and caregiver retention. This role promotes Talem Home Care & Placement Services, focusing on developing referral networks within healthcare, senior care, and community organizations. Success requires empathy, consultative sales skills, and comfort meeting families in their homes.

Key Responsibilities

  • Grow referral sources: hospitals, physicians, assisted living, skilled nursing, home health, hospice, senior centers, and community organizations.

  • Conduct all phases of consultative sales: cold calls, follow-ups, client visits, and recommendations.

  • Track leads and referrals in CRM (Salesforce).

  • Collaborate with Care Coordinator/Scheduling Supervisor to place caregivers with new clients.

  • Meet/exceed monthly referral and new client goals.

  • Develop co-marketing opportunities with senior-focused businesses.

  • Partner with SEO/social media teams to maximize lead generation.

  • Conduct formal group presentations and maintain referral provider relationships.

  • Keep accurate records of all sales, activities, and outcomes.

Qualifications

  • High school diploma required; bachelor’s in Marketing, Public Relations, or healthcare preferred.

  • 2–3 years in marketing, sales, or healthcare relationship management.

  • Home Care, Home Health, or Hospice experience a plus.

  • Valid Colorado driver’s license and auto insurance.

  • Ability to lift 30 lbs.

Compensation & Benefits

  • Base salary: $70,000 – $90,000 + commissions.

  • Incentive/commission structure.

  • Full-time, Monday–Friday, 8 AM–4 PM. (Flex schedule as needed)

  • Travel within Broomfield, CO territory (not remote).
