Position Summary
Wrapped N Love Home Care is seeking a Community Outreach & Intake Specialist to drive referral growth, strengthen community partnerships, and oversee the intake and on-boarding process for new clients.
This is a field-based, boots-to-ground role combined with structured intake coordination. The ideal candidate is relationship-driven, performance-oriented, and comfortable working in a KPI-focused environment where results, responsiveness, and execution matter.
This role bridges community outreach and operational on-boarding — ensuring that referrals convert efficiently into active starts of care while maintaining a smooth and professional client experience.
Key Responsibilities
1. Community Outreach & Referral Development
• Build and maintain strong relationships with referral sources including hospitals, facilities, social workers, case managers, and community partners.
• Conduct in-person outreach visits to expand agency visibility and referral opportunities.
• Develop and grow new referral partnerships (“power partners”) within assigned territories.
• Maintain consistent follow-up and engagement with existing referral sources.

• Serve as the primary point of contact for incoming referrals.
• Respond quickly to new referral inquiries and initiate outreach to families or representatives.

• Manage follow-up cadence to improve referral-to-start-of-care conversion rates.

3. Intake & Client Onboarding
• Conduct intake assessments and onboarding visits (including Medicaid and transfer clients).
• Ensure all intake documentation is complete, accurate, and compliant.
• Coordinate collection of required authorizations, consents, and service agreements.
• Prepare completed onboarding files for handoff to the care coordination team.

4. KPI & Performance Execution
• Track and manage performance metrics related to response times, conversion rates, intake completion, referral source growth, and contribution to billable hour growth.
• Maintain accurate real-time tracking and documentation within company systems.
• Participate in weekly KPI review discussions and performance evaluations.

Compensation & Incentives
• Hourly compensation based on experience
• Eligibility for discretionary performance recognition bonuses tied to KPI performance (not commission-based)

Compensation: $21.00 - $24.00 per hour
