Old Glory Home Health Care & Assistive Care is looking for an experienced Clinical Manager to help lead our growing skilled home health team. We are a physician- and nurse-owned, clinician-led agency serving patients throughout Central Virginia, and we are seeking a leader who believes home health should be done the right way — with compassion, accountability, communication, and excellence.

About the Role

The Clinical Manager provides day-to-day clinical leadership for the skilled home health team, supporting nurses and therapists in delivering high-quality, patient-centered care. This role helps ensure care is coordinated, documentation is accurate and timely, regulatory standards are met, and clinicians feel supported in the field.

The ideal candidate is a confident nurse leader who understands home health operations, enjoys problem-solving, and can balance clinical excellence with practical team support.

Key Responsibilities

  • Provide clinical oversight and support to field staff
  • Review patient care plans, documentation, and clinical concerns
  • Support admissions, recertifications, resumptions of care, transfers, and discharges
  • Assist with OASIS accuracy, care coordination, and regulatory compliance
  • Guide nurses and therapists in managing complex patient needs
  • Communicate with physicians, referral partners, patients, families, and internal team members
  • Help monitor quality outcomes, documentation timeliness, and patient satisfaction
  • Participate in QAPI, clinical education, performance improvement, and staff development
  • Support onboarding, mentoring, and ongoing training of clinical staff
  • Promote professionalism, teamwork, accountability, kindness, and high-quality care

Ideal Candidate

We are looking for someone who is a strong communicator, natural team leader, organized, dependable, calm under pressure, and passionate about mentoring clinicians. The right person understands home health care, values compliance and documentation, and knows how to lead with both accountability and compassion.

Requirements

  • Active Virginia Registered Nurse license required
  • Home health experience strongly preferred
  • Prior leadership, supervisory, case management, or clinical management experience preferred
  • Strong understanding of OASIS, care planning, documentation, and regulatory compliance
  • Excellent communication and organizational skills
  • Valid driver’s license, reliable transportation, and ability to travel as needed
